The facts and circumstances of each individual's case must be thoroughly explored. However the following forms will give you an idea as to what kinds of questions you should ask yourself or those we may ask about you and your problem.

Each office may have their own process of handling intake. We provide information and forms to provide a better understanding of the application process. You’re welcome to fill out them out, but please understand the office may not be able to get you in to see an attorney right away.
General Application - Everyone applying for Legal Aid needs to fill out this form.
If you see other forms that are relevant to your situation, please print and fill them out to bring with you to the LANWT office.

Land Lord / Tenant Questionnaire - If you have been served or received a Petition for Eviction, or have any legal problem concerning housing, including Section 8 Housing Assistance.
Parent Child Relationship Questionnaire - If you wish to file a Suit Affecting the Parent-Child Relationship, or if you are seeking to modify an existing Order in a Suit Affecting the Parent-Child Relationship.
Pauper's Affidavit - A pauper's affidavit is a sworn statement stating a person does not have sufficient funds to pay court costs for the filing of a new civil case. A judge must enter an order determining poverty.
